Do not complete your enrolment until you have read and understood the Agreement. By entering your initials above, completing your enrolment and selecting the checkbox below, you confirm that you have read, understood and agree to be bound by the Participation Agreement. A copy will also be emailed to you following enrolment for formal signature.
